SP+ Membership Agreement
This Membership Agreement (“Agreement”) is entered into by and between Sanctuary Pediatrics (“Practice”) and the parent/guardian (“Member”) on behalf of the patient/ on the date of signature below.
By enrolling in this subscription membership, Member acknowledges and agrees to the terms outlined herein.
SP+ Membership Benefits
As a Sanctuary Pediatrics Member, your family receives the following included benefits:
Unlimited provider visits – No additional visit fees for member child. Included services are at the discretion of Sanctuary Pediatrics.
Sports & club physicals included – At no additional cost.
Premium Care Services ($10/month per patient INCLUDED):
Premium Patient Portal Access: View your child’s medical records, eLab results, medications, documents, growth charts immunizations, and more. Anytime.
Enhanced Communication: Direct digital messaging (texting) and secure provider messaging (provider initiated)
Balance & Payments: View up-to-date balances, get itemized charge receipts, and make payments when necessary (SP+ Members use Member Billing Portal)
Sharing & Access: Enable authorized guests access, download patient documents
Patient Education: View and download patient education from your provider
4th child and beyond FREE – Membership covers additional children beyond three at no extra charge.
Members-only events & parties – Access to special gatherings hosted by Sanctuary Pediatrics.
Invitations to community S.E.R.V.E. events – Opportunities to participate in service-oriented community programs.
Monthly giveaways – Eligibility for practice-hosted drawings and perks.
Membership Fees
Membership is billed on a subscription basis, either monthly or annually:
SP+ Seedling (Newborn-3y) Membership: $99/month
SP+ Sapling (4y-21y) Membership: $59/month
Additional Children (4th+): FREE
Rates are subject to change with 60 days’ written notice; adjustments will not apply during the Member’s initial one-year minimum commitment period.
All fees are automatically charged to the payment method on file every month from the date of enrollment. Payments are non-refundable except as required by law or approval of ownership.
Term & Renewal
Membership requires a minimum commitment of one (1) year from the date of enrollment.
Payments may be made monthly, but membership may not be canceled before the completion of the first 12 months, except as provided in Section 6 (Early Termination Exceptions).
After the first year, membership automatically renews on a month-to-month basis unless canceled by Member with 30 days’ written notice.
Membership fee adjustments, if any, will only take effect at renewal.
Exclusions & Limitations
This membership is NOT health insurance and does not cover:
Emergency room visits or urgent care visits
Care outside of Sanctuary Pediatrics.
Specialist consultations, hospitalizations, or procedures performed outside the practice.
Vaccines, laboratory tests, or medications (unless otherwise specified).
Members are strongly encouraged to have and maintain a separate coverage plan for services not included in this membership.
Termination of Membership
Standard Termination
Membership may be terminated:
By Member, after completion of the 1-year minimum commitment, with 30 days’ written notice.
By the Practice, for non-payment, misuse of services, or inappropriate conduct, with written notice provided.
Early Termination Exceptions
A Member may request early termination of the Agreement prior to the end of the 1-year minimum if:
The family relocates outside of the service area of Sanctuary Pediatrics.
The Member experiences significant financial hardship that prevents continuation of membership.
The Practice and Member mutually agree to an early termination.
All early termination requests must be submitted in writing and approved by Sanctuary Pediatrics.
Legal Disclaimer
Memberships do not provide health insurance and should not be used as a replacement for a proper health coverage plan.
Sanctuary Pediatrics does not guarantee coverage for medical services outside the scope of this membership.
Membership fees are for participation in the program and access to listed services only.
Member is responsible for all healthcare expenses incurred outside of this membership, including but not limited to hospitalizations, emergency care, specialist referrals, imaging, and prescriptions (unless otherwise specified).
This Agreement is governed by the laws of the state in which Sanctuary Pediatrics operates.
Acknowledgment
By purchasing a Membership, Member acknowledges that:
Membership provides access and services but is not a substitute for health insurance.
Membership requires a minimum one-year commitment, except in cases of approved early termination.
Membership fees will remain unchanged during the initial one-year commitment period.
Member is responsible for maintaining separate insurance coverage as needed.
Member has read and understands the terms of this Agreement.
