Book an appointment.

    1. NAME & BIRTH DATE: When booking an appointment BELOW, your child is the patientand the one to whom ALL questions pertain. DO NOTuse parent/guardian NAME & BIRTH DATE unless specifically indicated(ie emergency/guardian contact).

    2. New Patients: If you are new to our office, please check the box next to “I’m a New Patient at this practice,” then choose New Patient (Healthy or Complex) appointment from the drop down.

    3. Forms: Most appointments require specific forms which will be sent AFTER your visit is confirmed by our office. After your INITIAL appointment you will also receive a link to set up your child’s Patient Portal (optional).

    4. Returning Patients: If you have already established care at Sanctuary Pediatrics, please select the “Returning Patient” box then choose your appointment type.

    5. Telehealth Visits: will be changed to in-office visits unless your Provider has specifically given you permission to schedule a Telehealth visit.

    6. SP+ Members: Select “No Insurance” as your insurance type. (except for SP+ add-on).

SICK VISITS: Please Call for same/next day sick appointments. We reserve sick visit time slots each day that are not available to book online (below).
